About Cape Atlantic INK's Behavioral Health Home
Cape Atlantic Integrated Network for Kids, Inc. (Cape Atlantic INK) will be offering a new component to its current design. The Behavior Health Home (BHH) model is scheduled to come to Atlantic and Cape May County families January 1, 2016.
Cape Atlantic INK, the Care Management Organization for Cape May and Atlantic counties, will integrate the BHH services into the youth's Individual Service Plan (ISP), providing a comprehensive system of care. We will now include children and adolescents with chronic medical conditions along with their mental health, intellectual/developmental, and substance abuse challenges.
The BHH is a person centered medical home that is able to fully address the medical, behavioral health, and service needs of individuals with chronic conditions.
BHH providers will integrate and coordinate all primary, acute, behavioral health, and long term services and supports to treat the "whole-person" across the lifespan. Cape Atlantic INK will be hiring a BHH Nurse and a Wellness Educator who will become an integral module of the Child Family Team (CFT). The impetus of the BHH model will be improving self-management and wellness that can be delivered in a variety of models.
The BHH module expands the service array and targets specific populations with Chronic Medical Conditions:
- Obesity (BMI at or above 85th percentile)
- Diabetes Mellitus
- Asthma
- Eating Disorder
- Developmental Disabilities
- Cystic Fibrosis
- Sickle Cell Disease
- Hypertension
- Kidney Disease
- Substance Use Disorder
- Developmental Disabilities
- Cardiac Disorder (requiring the care of a cardiologist at least yearly)
- Hypothyroidism
- Hyperthyriodism
- Polycistic Ovarian Syndrome
- Pre-Diabetes
- Autism with GI Concerns (GERD, constipation, diarrhea, IBS)
- Chronic Migraines
